Italy Visa Checklist

PASSPORT: Valid Passport, original + 1 copy of pages 2 and 3 only (i.e. the pages containing only the passport data and personal data) The passport must be valid for at least 3 months after the planned departure date from the Schengen area. The passport must have at least four blank and unmarked visa pages.

PHOTOGRAPH: Four recent and identical photos. Photos size must be (4.0×3.5cm) color photo with white background, taken within the last 6 months.

COMMON REQUIREMENTS FOR

APPLICANT:
 Tin Certificate.
 Persona Income Tax certificate (ITR) for the last 3 years [If available need to provide] Bank statement & Solvency certificate (last 6 Months-Personal).
 National ID Card / Birth Certificate (Applicant)
 Marriage certificate (if married mandatory).
 Last Education Certificate.
 Spouse and children’s passports or identification docs such as birth certificates or Nid
 Parent’s National ID Card / Birth Certificate

PROFESSION- BUSINESS:
 Trade license (1st and last one-notarized)
 Visiting card/ Business Card.
 Company blank pad/ letterhead.
 Company Memorandum/ Partnership Deeds
 Company Bank Statements: If the firm supports the trip, a copy of the sponsoring company’s or party’s bank statements for the previous six months clearly specifies the bank’s name and phone number with an approved bank stamp and signature.
 Company ITR Certificate: The applicant must produce the last three years’ Income Tax Return Certificate from the employer for which he or she works. If the firm is sponsoring the vacation, the license must be approved by legal authorities.

PROFESSION-EMPLOYEE/ADVOCATE/DOCTOR:
 Visiting card/ Business Card.
 No Objection Certificate (NOC) letter
 Office Identity Card (ID Card)
 Joining Letter, the date of hire

 Job Designation / The Desk position
 Salary Certificate /Pay Slip
 BMDC Certificate (For Doctor)
 Bar Council Certificate (For Advocate)

OTHER FINICAL DOCUMENTS:
 Land properties documents. (Need to provide asset valuation if available)

 Car documents. (if available) bluebook photocopy of the car if any
 House Rent /Tenant agreement (if you are receiving any rent) (if available)
 Personal FDR/DPS/MSS/Credit Card & Other Bank Documents ((if available)

STUDENT DOCUMENTS:
 Certificate from Alma meter
 NOC from Alma Mater
 ID card from an educational institution

IN CASE OF MINORS APPLYING FOR A VISA:
Consent of both parents or custodian if both parents are not traveling together Photocopy of passport of parents or legal guardian Proof of financial means of parents or legal guardian (see point 6) Original birth certificate of the minor issued by the relevant authority The status of “legal guardian” must be proven by documents issued by the relevant Bangladeshi Courts for minors

IF VISITING FAMILY OR FRIENDS:
Proof of sponsorship including proof of family/ties with a sponsor, and letter of invitation, specific countries may request additional supporting documentation, (see list of country websites for details)

TRAVEL INSURANCE:
Insurance must be valid for the whole Schengen area. The insurance must cover medical fees, hospitalization, and repatriation costs up to 30,000 euros. The insurance must fully cover the whole period of stay within the Schengen area. (Original & Photocopy)

MARRIAGE DOCUMENTS –CIVIL STATUS:

Marriage certificate (e.g. “Nika Namah” for Muslims or marriage act for Hindus, both translated into English) For divorced applicants: divorce certificate (e.g. “Talak Namah” for Muslims in English) For widowers and widows: death certificate of the spouse in English

AIRLINE TICKET ITINERARY:

A reserved itinerary for the applicant’s plane ticket based on the travel schedule and return from the trip.

HOTEL RESERVATION:

A planned schedule for the applicant’s lodging. He or she must specify the hotel or resort based on the trip’s itinerary.

If missing documents are not submitted within one day after submission, an application might be decided on the existing documents submitted.
